Great home visits! Home visits are most helpful as we have disabilities. Caitlin and Marie have been very thorough and supportive and pleasant and the office is most helpful with appointment reminders.

Excellent follow up with my Nurse Practitioner which covered recent blood work results and planning concerning diet, cholesterol, and blood sugar (type 2 diabetes). Also covered discussion of appointments with current medical specialists, e.g. Endocrinologist, Psychiatrist, and Ophthalmologist. Nurse Practitioner also conducted questionnaire covering my mental condition. Follow up appointment was also discussed.

Age well has been such a huge blessing for us. Having someone come to house and see my mom for PCP care is so helpful as my mom lives with me and I work full time. 24 hour access has been great when something comes up, of course, over the weekend and an urgent care appointment can be avoided. The medical assistants and nurse practitioners are so kind and caring. Thank you!

Baptist AgeWell has been incredible with my aunt, who has been bedbound for nearly two years. They are very communicative with me (as her medical proxy) and have given her such thoughtful care. I honestly don’t know where we’d be without AgeWell. Caring for our elderly loved ones is so difficult but so important, and I’m glad AgeWell is on our team.
