I’m giving a score of 4 for the following reasons. I don’t have a history of high blood pressure but my blood pressure was high during the visit and I had symptoms that could be related to high blood pressure. Yet, the finding of high blood pressure was not discussed with me, nor were any actions taken. I only found out my blood pressure was high by reading the after visit summary after I got home. Now I need to schedule another visit to get my blood pressure checked again!
