My experience was great. However, I don't think the injury was my ankle. It was the mid part of my foot. But other than that, the doctor was great. Everyone was great. He really knows what he's talking about, but I just don't think the injury was with my ankle. Thank you. Bye.

When it's my first visit there and everyone was very nice and cordial to me that I greatly appreciate that with the conditions that I was in with my foot and the pain, Dr. Zak was extremely kind and he had well manners and attended to my needs and answered my questions and I greatly appreciate that. But everyone there was great and nice and the office was clean and very pleasant visit and appointment that I had there and I'm going to continue my service, I mean getting my treatment there.

Hi, I was really impressed with Dr. Zak because I have been having this problem with my feet for a while and I think I'm in the right place. Hopefully the pain gets over. He was professional and understanding at the same time. I am having a pending parauthorization with him so that I can get started with my medication. If that is taken care of, it would be absolutely wonderful. Thank you.

Dr Zak was very nice but I don't feel he really addressed the issue appropriately. I was not aware of a $30.00 copay for an xray, in addition to the $30.00 copay for the office visit. This should be explained prior to being seen.

This was my first meeting with Dr Zak and I found him to be compassionate, professional and patient. He listened to my concerns/questions, provided me with detailed answers and setup a comprehensive plan for treatment. I would not hesitate to recommend Dr Zak to family and friends.

Everything went well but did not like driving 40 minutes for a 5 minute paper signing. I think they could have sent the papers to a closer office.

This visit was a very long process for me to make due to the fear and uncertainty! Dr. Zak made me feel confident and secure in my decision to seek the medical attention that I need to hopefully get back to a good physical condition in my future life.

I was pleased that I was able to schedule an appointment on the day that I called. When I arrived I was taken to an exam room shortly after. Dr. Zak listened carefully as I explained my injury and after viewing the X-rays taken at an urgent care facility he arrived at a diagnosis which he then explained as well as what treatment I should do at home.

The provider didn't listen or hear me he just did what he wanted and barely gave me any eye contact he ran in and ran out I told him my pharmacy had changed from one to another he had it sent to old pharmacy and now when I had someone go to get my medicine it wasn't the medicine we agreed to so I feel he lies just didn't care for him I'm a person not a number of

I had a wonderful experience at my office visit and was happy to find Dr. Zak and Katy (sp?) at the office. The entire staff was very nice and the office was clean and very welcoming!

Everyone was really nice. I was a few minutes late, maybe five minutes late, and I called into the office to let them know. The young lady that answered the phone was really great in terms of checking in with the doctors to make sure that I still could be seen, in which I was very appreciative of that, that I was able to be seen. As soon as I got in, I was able to get in. And the doctor came in, he, someone else did come in to greet me and take me to the room. She was really nice. And then the doctor, Dr. Vet came in and he didn't think that he would have been able to help me, but he tried anyway, which I'm very thankful for because he resolved the problem like really fast. And he was understanding of my situation. He didn't hurt me at all. And it was just a really good experience. And I'm really glad that he was able to help me because I thought I had

I've been telling the doctors they could not figure out what was wrong with me. This gentleman looked at my x-rays and he knew exactly what was wrong. So, I thank God for that and him too. Thank you.

It felt like I was an inconvenience to my provider because my issue was smaller than others usually come in with. I ended up agitating my injury after my appointment due to the poor advice my provider gave me. I left the office feeling more pain than when I came in.

I was impressed from the minute I walked in the lobby. The receptionist was very sweet, very cordial, and we could make her smile, and she was a very nice person. And the girl that took me into the office was sweet and thorough. And the x-ray technician, she was fantastic and very helpful. And I was really impressed with Dr. Zak. I wouldn't let anyone else touch my face. injury. So I'm really looking forward to him taking care of me. He seems to be the best and wonderful doctor I've met. And I thank him. And the crew. Everybody was nice.

Dr. Zak and his staff were exceptional! Everyone was professional, courteous, kind and cheerful. Everyone provided me with the best care. Dr. Zak took his time examining me and explained everything that was going on with my foot. He gave me options and was very knowledgeable, especially skilled in what he does. It was a pleasure to have meet Dr. Zak. And his staff very professional able to answer your questions assisted with documents for my job that was a big lift off my shoulder, now I can concentrate on my surgery instead of paperwork for my job. I referred a family member to Dr. Zak. he could not believe the quick respond once again thank you
